Virginia HOSA is instituting a new, Comprehensive Form system.  Instead of requiring students and advisors to complete Medial Liability and Code of Conduct forms for each HOSA event they attend, we will now offer ONE form that will apply to all HOSA activities throughout the entire school year.  

Once a member completes the Comprehensive Consent Form (3 signature areas in total), they will turn it into their Chapter Advisor.  

Chapter Advisors are responsible for keeping these forms and bringing them on-site to each HOSA event.  Chapter Advisors will NOT turn Comprehensive Consent Forms into VA HOSA.  Instead, they will maintain records to ensure that every participate at each event has a form on file.

Chapter Advisors will complete and turn in the Chapter Certification Form at each HOSA event they attend.  *Pro Tip: make a copy of this completed form so that you only have to do the work once (all you will have to do is change the name of the event)!
